PLEASE PRINT THIS PAGE FOR YOUR REFERENCE WHILE COMPLETING THE APPLICATION. Writing Sample All MFA and MA applicants must also submit a writing sample along with their photograph. This sample can be a previously written paper on any subject and does not have to be dance related. Please upload your writing sample into 'Document 1.' Audition Form Download and fill out the following file: mfa-audition.pdf. Upload 'audition.pdf' into 'Document 2.' Photograph All applicants must submit a full body photograph of yourself in practice clothes. The photograph is not used as a criterion of admission; it functions as a memory aid for the adjucdication committee. To upload your photograph it must be under 500 kilobytes if you are unable to upload the photograph please mail it to the address below. Upload your photograph into 'Document 3.' *Optional* Graduate Financial Aid If applying for assistantship download and fill out the following file: mfa-ma-gradasst.pdf. Upload 'mfa-ma-gradasst.pdf' into 'Document 4.' If applying for assistantship, please request three letters of recommendation and have them sent to the address below.
